New Prospective Student Evaluation Forms

New Students applying to our school must submit the Prospective Student Evaluation forms along with thier application. It must be completed by the current school your child is enrolled in and must be returned directly to our office for consideration for enrollment. It should not be given back to the parents for submission. Please find your copy for the appropriate grade(s) you are applying for: ​K4-1st​ & ​2nd-6th​.
